Alexander's allies have restrained several mutated Klingons in a medical facility, but their strangely twisted physiology has made examining them difficult. Study these patients to find the cause of their affliction. The away team conducts microcellular scans on the patients and finds that their bodies are still mutating at a slow rate. Working backwards from affected cells may lead to answers about what caused the mutation. Every lead the away team finds turns out to be a mere quirk of the Klingons' new physiology. They have no idea what caused these particular Klingons to mutate while their neighbors remain perfectly healthy.

Medicine Science Klingons' general disdain for doctors means there are few medical records of historical diseases or epidemics. Search the scattered archives throughout the First City for anything relating to the current mutations. The away team hunts down every half-collapsed archive or corrupted database in the First City. They find no historical outbreaks similar to the current mutations, suggessting that they were caused by a new, though unknown, factor. Although there may be more medical archives elsewhere, the away team only finds two whose databases are not corrupted beyond repair. They will have a hard time comparing this outbreak to any historical ones.

Security Arik Soong's knowledge would be invaluable in finding a cure for the mutated Klingons. Their past interest in kidnapping has made him reluctant to help them, however, unless he can be provided with a personal guard. The away team sets up forcefields and checkpoints around Soong's laboratory and living quarters. Although he calls them primitive, Soong feels safe enough to work on a cure for the mutated Klingons. The away team offers to guard Soong themselves, but after skeptically looking them over, he is not reassured: No matter how they argue with him, Soong insists he was safer in prison than he would be on Qo'noS.

Klingon fears of the Augment virus run deep, and the families of mutated Klingons are afraid that a trial treatment will leave their relatives ridgeless. Walk the families through the procedure to reassure them. The away team keeps their explanation short and to the point, and promises to let the Klingons' families observe the procedure. They nervously agree, and their mutated relatives begin showing signs of recovery. The away team tries to reassure the mutated Klingons' families by walking them through every detail of the treatment, but their confusion quickly turns to anger. Every family refuses the proposed treatment.

Antaak has developed a possible treatment for the mutated Klingons, but he wants to make sure there are no unintended side effects. He's asked you to give the medicine to a mutated targ, if you can catch one. After failing to lure the mutated targ into close range, the away team tries a different approach. They beam the targ into a holding cell and diffuse Antaak's cure into it. Within a few hours, its mutated growths and ridges seem to be shrinking. Even domesticated targs are not docile beasts, and the mutated wild targ the away team tries to catch is downright ferocious. They return to Antaak with his treatment, untested, in hand, and needing medical attention themselves.

Klingon doctors want to know whether their brethren's mutations are connected to the actual monsters on Qo'noS. As doctors, though, even Klingon ones, they cannot capture a kos'karii themselves. The away team members wear earplugs to avoid the kos'karii's deadly whispers as they fish one out of a river. They transport it back to the Klingon doctors for study in a reinforced, soundproof tank. Overestimating their own willpower, the away team goes fishing for kos'karii unprotected. The serpents' insidious whispers send half of the team diving into the water where the hungry kos'karii await.

Klingon hunters have surrounded a pack of monsters, ones that could be used to study this mysterious new affliction if kept alive. You'll need a good excuse to convince them to give up their prey. The away team tells the Klingon hunters that Fek'Ihr himself has been spotted nearby, a foe that would make any Klingon who slew him a legend. While the hunters race off after this rumor, the away team leads the monsters they've captured away. The away team offers the hunters furs, coin, and a barrel of bloodwine for the pack of monsters they've caught, but they only laugh and insist that none of that is worth as much as the monsters' own pelts will be.

Engineering Sirella, Martok's wife, has been restless since he disappeared, but now her servants say she won't leave her room. They're worried she is mutating like the others, but they cannot get past the shield around her quarters. The away team shuts down power to the house and transports into Sirella's room just before backup generators activate. Though irritated by the intrusion, Sirella is safe, merely trying to keep potential contagions out of her quarters. Deactivating the forcefield around Sirella's quarters only reveals a backup forcefield in place behind it. Sirella is a formidable woman, but the away team worries about what could be happening to her behind closed doors.

A group of Klingon hunters have mistaken Worf's mutated form for that of a monster. If he is not freed before they make it back to their camp with their prize, he may not survive the night. The away team ambushes Worf's captors in a ravine, some distracting the hunters while the others free Worf from a forcefield-surrounded cage. They beam out with him before the hunters can catch them. The away team tries to negotiate with Worf's captors, not realizing that asking a hunter to give up his rightful prey is the same as asking for a fight. After their scuffle with the hunters, Worf still remains a captive.

Mutated Klingons have overrun the Old Quarter of the First City, but the unique stone of its old buildings has made scanning for their whereabouts difficult. An away team will have to clear the streets one by one. The Old Quarter's winding streets allow several mutated Klingons to flank the away team, but they are prepared for this. The few Klingons they don't manage to stun, they trap in nearby homes while awaiting backup. Unsettled by the echoing howls of mutated Klingons and hemmed in by the Old Quarter's cramped walls, the away team only manages to clear a few streets before panicking and requesting an emergency beamout.

A massive pack of monsters has been tearing through the traps that hunting parties have set for them. The hunters need a better means of cornering them before the creatures reach nearby villages. The away team places bait in a small ravine and designs a duranium gate that slams shut once any monsters pass through it. The elated hunting party faces down the slopes of the ravine to face their prey in battle. The away team's traps corner a few of the beasts, but as the hunters arrive to fight their quarry, the improvised traps break, unleashing several very angry monsters right at the surprised hunters.

It takes three days and three nights of sailing before the away team closes in on the Serpent of Xol. They raise their bat'leths as it rears over the deck, and by the battle's end, the legendary beast is slain once more. The away team leads an exhausting voyage through the Sea of Gatar, but the only time they get close to the Serpent of Xol, it turns out to be an oddly shaped rock. Perhaps other hunters will have better luck.

Mutated saber bears are wandering the mountains of Qo'noS, with newly grown defensive plates that no spears can pierce. Hunting parties need a way to fell them before they reach the inhabited lowlands. Realizing there's no need for weapons in the right terrain, the away team lures the saber bears over the edge of the Kris'tak volcano's crater. It's not as glorious as a hand-to-claw fight, but it's undeniably effective. Every weapon that the away team crafts either splinters or bounces off of the mutated bears' hides. They beat an undignified retreat as a roaring pack of bears chases them down the mountainside.

When Klaa strikes Fek'lhr with it, the monsters falls to its knees. The away team constructs an enhanced disruptor for Klaa, but when he turns it on Fek'lhr, the blast bounces off it's hide. Only a shameful retreat saves Klaa from being torn apart by the monstrous being.

Sarpek the Fearless has earned his name once again through a series of daring hunts, but his rashness has left many of his fellow hunters wounded. They need medical attention before Sarpek leaves them behind. They are grateful to be able to rejoin Sarpek's hunt. The hunting party's wounds are severe, and Sarpek, impatient, rushes off to continue the hunt before the away team can heal them. A band of both Klingon and Hirogen hunters are fighting over who gets the trophies from their latest kill. Unless someone steps in to keep the peace, their prey may not be the only thing left lying on the ground. The away team examines the hunting party's fallen prey. Based on the wounds, they confirm that a Hirogen struck the last blow on the creature. The Klingons begrudgingly hand their trophies to the Hirogen. The away team has the hunting party describe the last moments of their hunt, but due to the heat of the fight, no one knows who struck the last blow on their prey. Duras, son of Ja'rod, has been using the chaos to masquerade as Molor reborn, but no disguise can turn a coward's heart into a warrior's. Everyone sees that he is a mere mortal, not the Molor of legends. The away team challenges Duras in public, drawing a large crowd of spectators. There are dozens of people watching when he easily defeats the away team, only furthering his claims that he is Molor reborn.
