Poisonings are part of the career advancement process in the Terran Empire, but now it seems Q has decided to play his own deadly game, swapping out intended lethal cocktails and making the simple act of hydrating a risky affair. Stop his treachery.
It would be impossible for your away team to check every beverage in the Terran Empire, but they manage to engineer a method that crews can use to check for themselves- O's concoctions turn out to be rather basic.
Unbeknownst to your crew, Q has seeded a few surprises in your ship's replicators - surprises that incapacitate members of your away team.

Crew members across the galaxy are changing on a genetic level! Is this merely Q's antics or is it another protomorphosis syndrome outbreak?
Donning proper protective gear and utilizing intel from the Enterprise—D's experience, you are able to treat the devolving crew members using an experimental synthetic compound- You distribute the details to other impacted crews.
Some of your away team members failed to take proper precautions and now you're left in charge of a team of ill—tempered reptiles.

As you scramble about the galaxy trying to set Q's chaos right, it seems certain individuals know your next move before you do. While at first this seems fine, as it is mostly higher-ups you would report to anyway, it quickly begins to feel uncanny.
You have your crew conduct a sweep of all equipment and address security weaknesses. When none are apparent, you begin to shift your crew around - and who you communicate with. That seems to slow down the reporting as your investigation continues.
Your away team is unable to find any concrete answers, but suddenly your higher—ups fall silent about any advance knowledge they have regarding your activities.

Your away team is able to devise a way to determine the original universe of all crews on prime universe ships and ultimately able to return all prime universe crews, even as some mirror crews flee to other parallel universes.
While dashing about the battlefield Q has created, your shuttle is destroyed by the emperor's forces and you are forced to send out a distress call.

While attempting to persuade one reluctant crew, your team is ambushed and you learn this isn't even a mirror universe switch; this particular swap has led the original crew members to be split across universes and timelines.
